8/9/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ben-Nita. Upon arrival to the home, Ben-Nita appeared to be in a relaxed mood evident by her facial expressions such as a smile on the face. QMHP-C inquired about her day. Ben-Nita stated to QMHP-C that her day was going well. QMHP-C informed Ben-Nita that this session was going to continue to focus on her making progress towards her ISP goals. 2. Behavior: During the second portion of the session, Ben-Nita appeared to be a little more understanding on how to achieve her goals on being able to learn how to communicate since the last session. She at times displayed hyperactive behaviors evident by her roaming around the room but once she was redirected, she was able to regain focus on the session. 1.Intervention: QMHP-C began session by inquiring about Ben-Nita current thoughts, feelings and emotions. QMHP-C assisted Ben-Nita with underlying factors that contribute to her negative anger-triggered feelings and thoughts. QMHP-C modeled effective ways with Ben-Nita and healthier ways to deal with siblings, peers and family members. QMHP-C engaged Ben-Nita in an activity to which consisted of Ben-Nita exploring periods of time when she has demonstrated good anger control and did not lash out of her grandmother, peers, and siblings and/or either become verbally or physically aggressive. 2.Intervention: QMHP-C discussed with Ben-Nita on the various methods that she has used before responding to others with aggressive behaviors. Throughout the session, Ben-Nita was taught effective communications skills and assertiveness skills to learn to express her feelings in a more constructive manner. 1.Response: Ben-Nita appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ben-Nita appeared to be excited about engaging in the therapeutic activity that was provided to her; however, Ben-Nita was redirected and prompted to discuss her negative feelings thoughts. After being redirected, Ben-Nita was able to get herself together and focus. Ben-Nita expressed that she has maintained good anger control and has walked away from situations. 2.Response: Ben-Nita recognizes her frustrations and was able to demonstrate for this QMHP-C how to be more assertive with expressing her feelings and thoughts.
